Community Health Worker/Patient Navigator Survey

The Health Council of South Florida (HCSF) in partnership with the University of Miami/Clinical & Translational Science Institute (CTSI), and the Florida Community Health Worker (FL CHW) Coalition, thank you for completing the 2019 Community Health Worker Census.   The collection of data is vital as it captures information that will assist in advocating for the CHW profession, training development, submission of funding proposals, and more. The goal of the Florida CHW Coalition will always be to champion for the advancement of the CHW profession.
