Community Health Worker/Patient Navigator Work Group
The MD-HAN Community Health Worker/Patient Navigator Work Group exchanges and disseminates best practices/lessons learned throughout the network contributing to the enhancement of service delivery among Community Health Workers (CHW) and Patient Navigators (PN).

The MD-HAN Community Health Worker/Patient Navigator (MD-HAN CHW/PN) Work Group of the MD-HAN represents the collective voice of Miami-Dade County’s CHW/PNs and is focused on bringing awareness to their critical work. The MD-HAN CHW/PN’s primary goal is to develop a platform for the exchange of best practices and lessons learned. This information is assembled and disseminated throughout the Network in order to contribute to the enhancement of service delivery among CHW/PNs. The efforts of the MD-HAN CHW/PN efforts are aligned with the Florida Community Health Worker Coalition, the statewide partnership dedicated to the support and promotion of the CHW profession in Florida.
The Work Group aims to accomplish the following objectives:

- Bring awareness to the work of CHWs/PNs in Miami-Dade County
- Promote a standardized a CHW definition and scope of practice
- Support and promote the statewide CHW certification initiative
- Identify and facilitate discussions on sustainable financing streams and reimbursement mechanisms
- Support the local implementation of CHW training programs and activities of the Florida CHW Coalition, Inc.

Community Health Worker/Patient Navigator Survey
​The Health Council of South Florida (HCSF) in partnership with the University of Miami/Clinical & Translational Science Institute (CTSI), and the Florida Community Health Worker (FL CHW) Coalition, thank you for completing the 2019 Community Health Worker Census. The collection of data is vital as it captures information that will assist in advocating for the CHW profession, training development, submission of funding proposals, and more. The goal of the Florida CHW Coalition will always be to champion for the advancement of the CHW profession.
Florida CHW Certification Process
The Certified Community Health Worker (CCHW) designation is an entry-level credential for front-line health workers who, by virtue of their trusted status in the community, serve as a liaison, link and intermediary between health services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Examples of job titles that are considered to be Community Health Worker positions include but are not limited to Promotores(as) de Salud; Community Health Educator; Health Communicator; Outreach Worker; or Health Advocate.
Note: The CCHW credential grandfathering period is now over; full certification application requirements must now be met in order to obtain the full CCHW Certification.
The full CCHW Certification requirements are:

- Formal Education: High School Diploma, General Equivalency Degree or higher
- Training: 30 hours of training in specified content areas
- Related Experience: 500 hours of related paid/volunteer experience
- Letters of Recommendations: 3 total; one supervisory recommendation, two
personal/character and/or supervisory recommendations
- Written Exam: Earn a passing score on the 100 multiple-choice exam

On Friday April 24, 2020, speakers Brendaly Rodríguez, MA, CPH from the University of Miami CTSI and the Florida Community Health Worker Coalition along with Julia Munroe, MS, Respiratory Disease Epidemiologist at the Florida Department of Health (FDOH), in collaboration with the University of Miami CTSI team hosted an Educational Session for Community Health Workers (CHWs) on the Novel Coronavirus (COVID-19). The speakers provided technical support to CHWs while recognizing the vital role they play in our collective response to COVID-19.
