HCSF Community Connection Program
The HCSF, in partnership with the City of Hialeah, is here to assist during this public health crisis. We understand that many residents have been affected by COVID-19 in our community and need access to health care and other social services now more than ever. The HCSF Community Connection Program serves as an access point to the City of Hialeah’s uninsured and low-income adults and children by providing outreach, education, assistance with enrollment into healthcare programs and linkages to social services. The goal of this program is to break down barriers that limit accessibility and engage with residents who potentially qualify for services. The HCSF Community Connection Program serves residents county-wide, with particular emphasis in high need areas such as the City of Hialeah.
The HCSF Community Connection Program:

Offers a team of Outreach Specialists that can provide FREE, unbiased, one-on-one assistance in connecting to and accessing social services.

Connects eligible residents in Miami-Dade County, City of Hialeah, to various social programs based on household needs, including; Medicaid, Supplemental Nutrition Assistance Program (SNAP), Temporary Cash Assistance (TCA), and Florida KidCare, Affordable Care Act health plans, and other social service programs.

Delivers advanced care planning information, resources, and support to members of the community and their families. For more information on advance care planning, check out our Conversation Hialeah program page.

Provides services in English and Spanish with translation services available in other languages.
